Loco can be purely clinical (mentally ill) or colloquially enthusiastic (estoy loco por el fútbol = 'I'm crazy about football'). Used as a noun, un loco / una loca means a madman/madwoman. In many Latin American countries it is also a very common term of address among friends, roughly equivalent to 'dude' or 'mate'.
