Cabecera has several concrete senses: the headboard of a bed (cabecera de la cama), the head of a table (sentarse a la cabecera), and in journalism or broadcasting, a masthead or title sequence (cabecera de un periódico). In medicine, médico de cabecera is a GP or family doctor. The underlying sense is always 'the head or top position' of something.
