Decisión is feminine and takes tomar as its standard verb: tomar una decisión ('to make a decision') is the idiomatic pairing — avoid hacer una decisión, which is a direct calque from English. The plural decisiones is regular. Decisivo (decisive) and decidir (to decide) share the same Latin root.
